This is the sample format of the Affidavit to regional transport authority for getting duplicate driving license

Affidavit

I, A, aged about ……………………… years s/o, W/o, D/o Shri………………… resident of …………solemnly affirm and declare as under:

1. That I am the deponent and have filed the accompanying application for the issuance of duplicate driving licence and I am fully conversant with the facts hence competent to swear this affidavit.

2. That I was issued driving licence No. ………………… dated …………. by this authority, to drive MLV/LV with/without gears.

3. That my driving licence No. ………….. has been lost/stolen on….. for which I have lodged FIR at Police Station, ………….. …………. vide FIR No. …………………….. dated ………………… U/s_______. the copy is enclosed.

4. That I have not deposited the said licence with any court or anywhere or my licence has not been cancelled. I have not been convicted of any offence by any court.

5. That no charge sheet has been filed against me for any offence under the Motor Vehicles Act or Rules made thereunder or any other law in force.

6. That I do not suffer from any of the diseases disqualifying me to hold the driving licence.

7. That if the original licence will be found, I shall deposit the same with the Office of Regional Transport Officer.

8. That in view of the above, it is necessary that a duplicate driving licence may be issued to me.

Verified on this _____________ day of ___________2015 that the contents the above affidavit are true and correct to my knowledge and nothing material has been concealed therefrom and no part of it is false.

    This is the sample format of affidavit. Keep in mind that it should be drafted on a Rs. 10 stamp paper. if given alone. otherwise it should be notarised.

        the above affidavit is to be drafted and submitted with the RTO (regional transport officer) and not before the police.
        you have to inform the police about the previous driving licence then get the FIR/NCR registered.
        then attach a copy of NCR/FIR with the affidavit

    My driving licence has been stolen. Now I not having photocopy of the same. how can I apply for the duplicate driving licence.

      you can file and application before the concerned authority to know the details of your driving licence.
      Or you can file an RTI to get a copy of the same from the concerned department.
      Thne you can apply for the duplicate DL.
